This is Warp Island. The 5th level is a minigame, Heavy Metal. The 10th level is a conveyor-belt level, and the 11th is the boss battle. In the boss battle, the player is no longer in Warp island, instead in a Pirate Seas style level, with the middle and 2nd lanes have planks.
